Peroneus Brevis Tendon - Longitudinal Split

The peroneus longus and brevis tendons wrap around the posterior aspect of the lateral malleolus. Splits or longitudinal tears may occur in the anteriorly sited peroneus brevis tendon at the level of the lateral malleolus as shown here. This is the result of pressure from the longus tendon, usually in association with acute trauma.
